Community Integration Counseling

CIC staff offer assistance, support, training and counseling which will ease the stress of the participant and family members in order to re-integrate and re-orient oneself into the community following a traumatic brain injury.  It is a service provided to a participant learning to cope with altered abilities and skills, the need to revise long-term expectations, and changed roles in relation to significant others.  CIC may also be available to anyone involved in a significant ongoing relationship with the participant, when the issues to be discussed relate directly to the participant. The primary work of the PBIS and CIC providers takes place in face-to-face sessions with the participant. 

 

Qualifications: 

Masters Degree in Social Work, Psychology, or related Human Service Field.  Minimum of one year providing adjustment related counseling to individuals with physical, developmental or psychiatric disabilities.

Position Summary:

Community Integration Counseling is an individualized service designed to assist individuals to more effectively manage stressors confronted when living in a community setting. This service works cooperatively with other Waiver providers.

 

Responsibilities:

* Assess individual’s needs and develop a structured goal plan, which will include a description of the individual receiving the service and future outcomes for the service. The focus of the plan will be person- centered and will target emotional and social adjustment to community living.

 

*Implement plan by meeting with the designated consumers as indicated in their service plan and fulfilling their therapeutic needs. Meeting can be made in individuals’ home, in the community or in the provider’s office.

 

*Provide training as needed to all individuals involved in carrying out the goal plan i.e. caregivers, in- home workers, supervisors etc.

 

*Interface with the service coordinator and all other Service Providers to maintain communication regarding the progress of the person in need.
