Established in 2009, North Shore TBI Services, Inc. is a trusted human services agency that provides New York State Department of Health Waiver Program services to individuals all over Long Island, who are living with traumatic brain injuries and other chronic medical conditions.  North Shore TBI Services, Inc. is an approved provider of the following programs under the New York State Dept. of Health Home and Community-Based Medicaid Waiver.

SERVICES

Positive Behavioral Intervention and Support Services (PBIS)

PBIS services are provided to participants who have behavioral difficulties that jeopardize their ability to remain in the community due to inappropriate responses to events in their environments. The primary goal of PBIS services is to decrease the intensity or frequency of targeted maladaptive behaviors, and to promote more adaptive behaviors and responses to their environment.

Independent Living Skills Training (ILST)

ILST services are individually designed to improve the ability of the participant to live as independently as possible. ILST services may be provided in the participant’s home and/or in the community. ILST services may include, but are not limited to, assessment, training, and supervision of, or assistance with, matters related to self-care, medication management, task completion, communication and interpersonal skills, socialization, community transportation skills, money management, and problem solving skills.

Community Integration Counseling (CIC)

CIC is an individualized service designed to assist the waiver participant in effectively managing the emotional difficulties associated with adjusting to and living in the community. It is a counseling service provided to participants coping with their altered level of skills and abilities as well as with their changed roles in relation to significant others.

Environmental Modification (EMODS)

Environmental Modifications (E-Mods) are internal and external physical adaptations to the home, which are necessary to ensure the health, welfare and safety of the Waiver participant.  These modifications ensure that the Waiver participant will function with greater independence and prevent institutional care.  Allowable E-Mods in the home include the installation of ramps,  lifts, hand-rails or grab bars, accessible showers, tubs and sinks, and widened doorways.  When medically necessary, E-Mods may also cover heating and cooling adaptations, electrical wiring to accommodate medical equipment, or adaptations to provide for emergency fire evacuation.

CAREERS

Independent Living Skills Training (ILST)

  • Assessing, training, and supervising, or assisting a client with self-care, medication management,task completion, communication skills, interpersonal skills, socialization, sensory/motor skills,mobility, community transportation skills, money management, and ability to maintain a household

  • Developing a detailed plan describing for each client the expected outcomes, method or type of intervention planned, and the frequency and intensity of services being provided

  • Developing and submitting an annual treatment plan and progress report

  • Preparing and submitting bi-weekly case notes

  • Participating in agency-wide program training and/or related activities

  • Participating in program meetings to be held at least quarterly

Job Requirements:
· A Bachelor’s degree and two (2) years of experience developing or implementing ADL/IADL goals, and teaching individuals with disabilities or seniors to be more functionally independent
OR
· An Associate’s Degree and five (5) years of experience developing or implementing ADL/IADL goals, and teaching individuals with disabilities or seniors to be more functionally
independent

Qualified candidates can earn a starting salary of $20 to $22/hour

Traumatic Brain Injury Medicaid Waiver Program (TBI)

This program is for people with a traumatic brain injury and is designed to be a cost-effective alternative to nursing home placement which enables the participants to live in the least restrictive setting possible. The TBI waiver philosophy is to provide complete freedom of choice.

Nursing Home Transition and Diversion Waiver Program

The Nursing Home Transition and Diversion (NHTD) waiver uses Medicaid funding to provide supports and services to assist individuals with disabilities and seniors toward successful inclusion in the community. Waiver participants may come from a nursing facility or other institution (transition), or choose to participate in the waiver to prevent institutionalization (diversion). NHTD Waiver services are provided based on the participant’s unique strengths, needs, choices and goals. The individual is the primary decision-maker and works in cooperation with providers to develop a Service Plan. This process leads to personal empowerment, increased independence, greater community inclusion, self-reliance and meaningful productive activities.
